Regarding point 3 for cyclists: you should never invite another road user to do something. If you want the other person to go first, you should stop and wait without making any gestures. Looking them squarely in the eye so that they're aware that you know they're there will help. If you wave someone on, and they have an accident, who's going to be sued? I am pretty shocked actually at the motorcycle/scooter drivers here in Australia. They act like bikes, squeezing in between cars at red lights to get to the front etc. I've never ever seen that done in North America - motorcycle safety seems to be a bit stricter there.
